Generic forms
Some services use the generic forms rather than procedure-specific forms:

Consent for post mortems
Cambridge University Hospitals uses postmortem consent forms published by the Department of Health (DH) and given (with other DH information) to relatives by staff of Patient Affairs and/or Children's Services. The consent forms on the DH website are for information only. They are not for printing and use within the Trust because the forms are a legal document and specially designed in triplicate.
