Generic forms

 

Some services use the generic forms rather than procedure-specific forms:

 

Consent form (adults) - generic

 

Consent form (Child or Young Person) - generic

 

Patient/parental agreement to investigation or treatment (Procedures where consciousness not impaired and no tissue samples or photographs are to be taken)

 

Consent form for adults who are unable to consent to investigation or treatment

 

Consent to investigation or treatment by patient who refuses to have a blood transfusion

 


Consent for post mortems

 

Cambridge University Hospitals uses postmortem consent forms published by the Department of Health (DH) and given (with other DH information) to relatives by staff of Patient Affairs and/or Children's Services. The consent forms on the DH website are for information only. They are not for printing and use within the Trust because the forms are a legal document and specially designed in triplicate.
