Generic forms

 

Some services use the generic forms rather than procedure specific forms.

 

We will give you the patient information (below) to read before you sign the consent form. Please read this carefully as it gives you important information about consent and explains the terms used in the form.

 

Information for patients about consent

 

Consent form - patient agreement to investigation or treatment (for adults and children)

 

Consent form - patient agreement to investigation or treatment for neurosurgery, spinal surgery or vitreoretinal surgery

 

Consent form - patient agreement to investigation or treatment for neurosurgery, spinal surgery or vitreoretinal surgery

 

Supplementary consent to investigation or treatment by patient who refuses to have a blood transfusion

 

Form for adults to lack the capacity to consent to investigation or treatment including assessment of mental capacity

 

 


Consent for post mortems

 

Cambridge University Hospitals uses postmortem consent forms published by the Department of Health (DH) and given (with other DH information) to relatives by staff of Patient Affairs and/or Children's Services. The consent forms on the DH website are for information only. They are not for printing and use within the Trust because the forms are a legal document and specially designed in triplicate.
