Preparing for home

Neonatal Services

The prospect of finally taking your baby home, whether after months of being with neonatal services, or just a couple of days, can bring feelings of great anxiety as well as relief.  You may have become used to the high level of support and gained reassurance in knowing that there is always someone on hand. Going home may mean that you feel much more on your own. However, your baby is only going home because he/she is ready and you will be fully prepared to look after him/her following your stay in SCBU. The neonatal community support team will support you at home alongside your health visitor if you live within the area that they cover.

Discharge coordinator with parent and twins

Sara Harris is the discharge planning co-ordinator for neonatal services, and aims to ensure that the transition from hospital to home is as smooth as possible. Sara works in collaboration with other members of the multidisciplinary team, such as dieticians, the neonatal community support team, speech and language therapy to help and prepare you in caring for your baby through your journey towards home.  Sara works to provide support and education around the needs of your baby, some requiring more input than others, to ensure a stress-free transition towards home. She will discuss all areas of taking your baby home, safe sleeping position, getting the temperature correct at home, safe travelling and will liaise with your health visitor, along with the community support team to plan the extra support that you may need.
